Job Summary:

Under the clinical supervision of the Nurse Manager, the Day Center Nurse’s responsibilities include monitoring the health status of including day-center-only clients and Pace participants, assessing for medical need and performing nursing tasks while in the PACE center. This position communicates with the Health Center to coordinate medical care and services. The DC Nurse provides clinical oversight for the Day Center C.N.A.s to ensure the health and safety of the participants. Rotating Saturday coverage may be required.

  • Conducts histories, physical exams and functional nursing assessments of ADC attendees.
  • Understands the triage process: provides acute care, treatments, medication administration and counseling to participants/caregivers facing chronic conditions and end of life issues.
  • Knowledgeable in diabetic management: Administers insulin if Medication Nurse to ADC participants as ordered. Prepares and coordinate delivery of medications/prefilled insulin to home.
  • Recognizes individual differences and understands the basics of motivational interviewing.
  • Communicates and collaborates with the Health Center to provide emergent care, acute care and preventive care.
  • Provides nursing care and service to participants per plan of care and makes recommendations to IDT for services needed.
  • Engages in problem solving on a RN/clinical level. Is knowledgeable and adheres to all nursing policies and procedures regarding standards of care. Can identify appropriate resources when unsure of specific policies or procedures.
  • Communicates with families regarding delivery of services and changes in orders that will impact care.
  • Performs narcotic counts in conjunction with a Nurse/PCP or Manager/Director to ensure appropriate delegation of narcotic medications.
  • Provides clinical oversight and training of CNA’s. Reviews vital signs taken by CNAs for accuracy and communicates abnormal readings to PCP as indicated.
  • Monitors participant groupings in the PACE center for appropriateness (Dementia, cultural, functional, etc.) and makes recommendations to the CNA staff.
  • Participates in immunizations clinics and administers vaccines to participants and staff as needed.
  • Provides communication and problem solving to vendors with clinical/nursing issues.
  • Maintains and monitors medications in the STAT SAFE. Coordinates with pharmacy to refill as needed.
  • Assists with lab draws (in the center/home) and coordination of X-ray services if clinic staff is unavailable.
  • Assists with orientation of new staff and serves as a mentor to nursing students.
  • Understands the Adult Day Center Regulations pertaining to nursing to ensure regulatory compliance. Understand the difference between services provided and EMR documentation for Pace participants and Adult Day Center only clients.
  • Collaborates with ADC Team to create quarterly ADC-only clients’ care plans.
  • Updates monthly RN Substitute Binder.
  • Completes documentation of all nursing care and services in medical record. Records all nursing encounters per PACE documentation standards.
  • Uses proper universal and standard precautions and Infectious Control PPE according to PACE policy and procedure.
  • Protects privacy and maintains confidentiality of sensitive participant and agency information.
  • Attends staff and department meetings as scheduled and agency in-services as required.
  • Works rotating weekends and/or holidays as assigned.
  • May be requested to work at other PACE-RI sites or in the community.
  • Performs other related duties as required and assigned.

Education Requirement:

Associate's Degree, Required
Bachelor's Degree Preferred


Physical Requirements:

Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb. participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.


Experience:

2 Years of direct service work (Adult Day, Nursing Home or Home Care), Preferred
1 Year of experience in a Medical or Geriatric Care setting, Preferred
1 year of experience working with a frail or elderly population, Preferred


Licensure & Certification Requirements:

Rhode Island Registered Nurse License (RN)


Position Requirements:

CPR Certification
Driver's License & access to reliable transportation
