Coordinates continuity of care from acute hospitalization to a lower level of care based upon the patient's needs.
JOB SPECIFICATIONS
Education and/or experience: Registered Nurse in the State of Tennessee.
Knowledge: Transition of care, planning process including HEDIS. Surplus Gain Share Contracting, insurance regulations (Medicare and /or Commercial) as applicable to outpatient transitions of care.
Skills: Intermediate computer skills.. Good oral and written communication skills; organizational skills required.
Abilities: Ability to interact with patients/families and other healthcare disciplines to coordinate seamless continuity of patient care.
