Director of Intake and Admissions (RN with current TN nursing license, BSN preferred)

Status:8 a.m. - 5 p.m.

Days:Monday - Friday

Hours: 40/week

SUMMARY

Under the Director of Clinical Partnerships, the Director of Intake and Admissions serves as a clinical resource for referral sources, staff, liaisons and patients. Oversees day to day operations of referral to admission process. Functions at a level similar to the director. Represents the agency to all constituents in a professional manner and facilitates access to services using all available methods.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Proficient in liaison, admission nurse and intake functions and is expected to oversee and train others in these roles.

Gathers and relays pertinent clinical information to Home Team and IPU physicians to make appropriate level of care decisions.

Provides oversight and holds staff accountable for the efficient and accurate performance of these functions in conjunction with the director.

Functions as the director in his/her absence

Demonstrates flexibility and takes initiative to identify service opportunities and solves them including personally meeting the need.

Demonstrates the ability to prioritize and multi task while simultaneously providing excellent service to internal and external customers.

Directs others to be equally flexible and service oriented

Trains new staff in these roles and monitors competency of staff in these roles

Demonstrates excellent interpersonal skills (verbal, written and with body language) and capable of modifying demeanor and approach to the situation and stakeholder involved.

Participates in meetings and IDT representing the functions of the department and agency, policies, Conditions of Participation (COP) and other regulatory requirements.

Demonstrates initiative in problem solving and performance improvement to move the department forward with innovative solutions.

Other duties may also be assigned.

SUPERVISORY RESPONSIBILITY

Administrative responsibilities as assigned by the Director of Intake and Admissions.

CONTINUING EDUCATION

The agency requires this position is required to complete 15 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Registered Nurse with a current TN nursing license, BSN preferred. Knowledge of or willingness to learn hospice care. Minimum 5 years nursing practice or equivalent experience required in relevant setting

CERTIFICATES, LICENSES, REGISTRATIONS

Current Tennessee RN License. Prefer Hospice certification. If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee State Law.
