Operation Upward

Members will deliver case management for the services that are offered to individuals. Members will meet with individuals for one hour. The member will assist individuals with completing intake applications. After completion of the intake application, the member will interview and complete a case management support plan with the individual. Thereafter, the member will enroll the individual in one or more of our services. The member will follow up with the individual two times utilizing the interview method after the individual has been enrolled using the follow up case management support plan. The dosage of each intervention we offer are as follow: Job skills, life skills, financial literacy, nutrition, opioid and substance abuse/drug intervention, physical activities, and healthcare awareness and knowledge, GED, counseling/coaching, referral, and outreach interventions. Members will assist, train, educate, and/or refer participants in each area of intervention
